The annual ICD-10-CM update takes effect October 1. Practices that keep coding from last year’s tables into Q1 and Q2 invite edit failures and under-specified HCC capture. The 2026 cycle brought hundreds of additions, a smaller set of revisions and deletions, and several changes that commercial and MA plans are already enforcing in medical review.
Skip the ultra-rare tropical and research codes. Focus on the families that touch primary care, behavioral health, endocrinology, hospitalist notes, and any clinic capturing social determinants.
SDOH Z-codes (Z55–Z65) moved from optional to material
CMS expanded social-determinants reporting, and several large commercial and MA plans now treat documented SDOH codes as part of care-coordination and risk workflows when they sit beside chronic diagnoses. Capture is still clinical documentation first — codes cannot invent what the chart never said.
High-value examples to train on: Z59.41 (food insecurity), Z59.811 (housing instability, housed), Z60.4 (social exclusion and rejection), Z62.21 (child in welfare custody). They are not typically paid as standalone line items, but they support risk adjustment and quality reporting when used correctly.
Severe sepsis sequencing is less forgiving
R65.2- (severe sepsis) continues to require careful sequencing: underlying infection first when guidelines call for it, then the R65.2- code, then organ dysfunction. Coding R65.20 alone without the infection and dysfunction picture invites medical-necessity and DRG/outpatient edit problems as payers tighten review.
Diabetes: stop defaulting to E11.9
Unspecified type 2 diabetes without complications is still valid when the chart supports it — but documented cardiovascular and other complications require the more specific E08–E13 combination codes. Under-coding complications leaves HCC weight and medical-necessity support on the table.
Behavioral health F-code cleanup
Watch deleted or replaced unspecified depression codes. After the October effective date, retired unspecified options should not appear on claims. Use severity-specific F32.- episodic codes or F33.- recurrent codes that match the documented presentation.
Post-COVID sequelae specificity
U09.9 remains the parent idea for post-COVID condition, and newer sub-codes allow more precise sequelae (fatigue, cognitive impairment, dysautonomia, and related presentations). Specialty clinics billing cognitive testing or rehab related to post-acute COVID need that specificity for coverage alignment.
Before the next claim batch
- 1Confirm encoder and scrubber version dates reflect the current ICD-10-CM release.
- 2Audit 30 charts across your top specialties — pre- and post-update — for deleted codes and under-specificity.
- 3Brief providers on retired codes still appearing in favorites lists and templates.
- 4Stand up a minimal SDOH intake path so Z-codes can be captured when clinically documented.
Most of the codebook change is noise for any one specialty. Sepsis sequencing, diabetes specificity, behavioral health deletions, and SDOH capture are not. eBillRCM treats those four as mandatory briefing items every fall.
